We are talking about some kind of disease of the male genital organ (basar, flesh instead of pudenda, cf. Lev. 15:19; Ezek. 16:26, 23.20), but not about seminal flow (as in the Vulgate: qui patitur fluxum seminis). It is difficult to define a disease in terms of modern medicine.
